The clinic will be hosted at the Chaldean Community Foundation on Tuesday, November 15, 2022 from 1:00PM-6:00PM.
Please enter in through the back of the building and find the signs. Parking will be available in the front and side of the building. Signage will be there to direct you towards the clinic entrance.
Register today to claim your appointment. Please bring your photo ID, insurance card, and prescription card with you. Vaccines are available to anyone 3 years of age and up. Shots will be administered by CVS pharmacists.
